This page brings together clinically feasible avascular necrosis thesis topics for MS Orthopaedics covering aetiology, risk factors, MRI staging, conservative management, core decompression, joint-preserving surgery and arthroplasty. The designs are intended for MS Orthopaedics candidates and can usually be completed within one thesis period using patients, radiographs, MRI studies, operative records and functional assessments already available in a routine orthopaedic service. Strong projects define disease stage, lesion burden and the unit of analysis before recruitment, particularly when both hips are involved. A selected topic can then be developed into an avascular necrosis research topic for MS Orthopaedics, MS Orthopaedics avascular necrosis protocol and MS Orthopaedics avascular necrosis synopsis.

The topics above work as research questions anywhere — what changes is the document the institution expects, and who approves it before data collection begins. The Gulf equivalent of an Indian synopsis is the avascular necrosis research proposal submitted to an institutional review board, and it ordinarily carries three sections an Indian synopsis does not: a Gantt chart, a budget and resources section, and a Declaration of Helsinki statement.
Board and residency programmes — country by country
Saudi Arabia — SCFHS and the Saudi Board. Residency and fellowship training under the Saudi Commission for Health Specialties includes a research project with a set timeline, and the avascular necrosis research proposal is the document prepared at the outset and cleared by the institutional review board before recruitment starts.
United Arab Emirates — DHA, DOH Abu Dhabi and MOHAP. Residents training in Dubai, Abu Dhabi and the northern emirates prepare an avascular necrosis research protocol for their programme and submit it for institutional review board approval before any data are collected.
Qatar — DHP (formerly QCHP) and Hamad Medical Corporation. A avascular necrosis IRB proposal is reviewed before recruitment, with the ethics section written to the institution's own template rather than a generic one.
Bahrain — NHRA. Trainees turning avascular necrosis research topics into a project need the proposal cleared by their institutional research and ethics committee before fieldwork begins.
Oman — OMSB. Residency programmes under the Oman Medical Specialty Board include a research component, and the avascular necrosis research proposal is the document assessed at the start of it.
Kuwait — KIMS. A avascular necrosis study protocol goes to the institutional committee for approval before the project begins.
Arab Board programmes across the region. The Arab Board carries its own research requirement irrespective of the host country, and the avascular necrosis proposal follows the same structure throughout.
Postgraduate degrees — Malaysia, the Gulf and beyond
Malaysia — MMed, the National Medical Research Register and MREC. A avascular necrosis dissertation proposal for a Master of Medicine programme carried out in a Ministry of Health facility must be registered on the NMRR and approved by the Medical Research and Ethics Committee before the study begins, and the guidance asks for submission four to six months ahead of data collection. Every investigator on the study team registers on the NMRR as well, so the methodology, ethics and team sections are written in far more detail than an Indian synopsis requires.
PhD and Master's candidates elsewhere. University programmes generally require a full avascular necrosis research proposal of roughly 6,000 to 10,000 words, with an extended literature review, a theoretical framework and a detailed methodology chapter.
PhD and MMed proposals are written individually by a medical doctor and revised until the supervisor accepts them. They are not produced by the automated protocol generator.

Current research is moving towards better prognostic stratification and more rigorous evaluation of hip-preserving treatment rather than relying on stage alone.
Case definition and stage: define whether the study includes traumatic, non-traumatic or both forms of femoral head avascular necrosis and name the staging system used. Ficat-Arlet and ARCO should not be mixed casually because the categories are not interchangeable, and pre-collapse and post-collapse hips should not be pooled without justification.
Unit of analysis: state whether the study analyses patients or hips. In bilateral disease, two hips from the same patient share age, steroid exposure, alcohol exposure, systemic illness and many rehabilitation factors, so they are not statistically independent observations.
Imaging measurements: specify how lesion size, modified Kerboul angle, percentage head involvement, weight-bearing surface involvement, bone marrow oedema and collapse are measured, on which MRI planes, and by whom. If more than one observer measures them, reliability should be analysed separately from their clinical association.
Treatment pathway: for outcome studies, record why a patient received conservative treatment, core decompression, augmentation, grafting or arthroplasty. Baseline stage and lesion burden influence treatment choice and outcome, so an uncontrolled comparison of different treatments may mainly measure initial disease severity rather than treatment effect.
An avascular necrosis synopsis is the shorter document used for institutional or university thesis registration. It should state the research question, objectives, study design, setting, sample, eligibility criteria, principal variables, proposed analysis and ethical safeguards.
The full protocol should define the disease stage, imaging sequence used for classification, lesion-measurement method, handling of bilateral hips, clinical outcome instrument and follow-up interval in enough detail for the study to be reproduced. A protocol comparing treatment groups should also state the clinical criteria by which each treatment is selected.
For longitudinal studies, the protocol should distinguish symptomatic improvement from structural progression. A patient may report less pain while MRI or radiographs show progression towards collapse, so functional improvement and imaging progression should be prespecified as separate outcomes rather than treated as interchangeable measures of success.
Cross-sectional and prognostic studies: calculate sample size around the primary association or expected prevalence and define whether the denominator is patients or hips. If both hips from a patient are included, the analysis should account for within-patient clustering rather than doubling the apparent sample size.
Imaging studies: use kappa or weighted kappa for categorical stage agreement and an appropriate intraclass correlation coefficient for continuous measurements such as necrotic angle or lesion size. Correlation between two measurements does not by itself demonstrate agreement.
Outcome studies: predefine one principal clinical endpoint and one principal structural endpoint. Baseline stage, lesion extent, age, aetiology and bilateral disease are potential confounders. Time-to-event analysis may be more appropriate than a simple proportion when the endpoint is femoral head collapse or conversion to total hip arthroplasty and follow-up duration varies between patients.
Start with the part of the disease pathway your department sees consistently. Centres with reliable MRI access can study staging, lesion burden and predictors of collapse; centres performing regular core decompression can study stage-specific outcomes; and high-volume arthroplasty units can study recovery after total hip arthroplasty for advanced disease. Before registration, estimate how many eligible patients and hips are likely to be available within the recruitment period and how many can complete follow-up.
Feasible designs include cross-sectional clinical-profile studies, case-control studies of risk factors, diagnostic-accuracy studies, imaging-correlation studies, interobserver agreement studies, prospective or retrospective cohorts, prognostic studies and comparative observational studies of routinely selected treatments. Randomised trials are possible only when genuine clinical equipoise, adequate recruitment, standardised intervention and reliable follow-up are available.
Agree on the aetiological group, staging system, imaging protocol, lesion-measurement method, unit of analysis, treatment category, functional score and follow-up interval. If bilateral hips will be included, decide before registration whether both hips will enter the analysis and how statistical dependence between them will be handled.
That depends on the question, but mixing stages without a clear analysis plan is usually a mistake. Prognostic and hip-preservation studies are often most interpretable when restricted to pre-collapse disease or stratified by stage. Arthroplasty studies naturally involve advanced disease. If both are included, the protocol should define stage-specific objectives because predictors, treatment options and expected outcomes differ substantially before and after femoral head collapse.
The synopsis is the concise registration document, whereas the protocol is the detailed operational plan. For avascular necrosis, the protocol should specify how the diagnosis is established, which staging system is used, how MRI lesion burden is measured, whether one or both hips contribute data, how treatment selection is documented and which clinical and structural outcomes will be assessed.
No radiograph, CT examination, MRI sequence, contrast administration, venepuncture, core decompression or other intervention should be added solely for research unless it has separate scientific justification and ethics approval. Prospective participants require informed consent. If younger participants are enrolled, guardian consent and child assent from about seven years should be obtained where developmentally appropriate. Retrospective record or PACS studies may be eligible for a consent waiver if approved by the ethics committee.
Imaging datasets must be anonymised because DICOM headers and exported images can retain identifiers. The protocol should define how clinically important incidental or unsuspected findings are handled, particularly asymptomatic contralateral osteonecrosis, subchondral fracture, early collapse or another significant hip abnormality detected during research review. Separate consent is needed for identifiable clinical photographs or operative video used beyond routine care.
Not without accounting for clustering. The two hips belong to the same patient and therefore share patient-level exposures such as corticosteroid use, alcohol consumption, systemic disease, age and many rehabilitation factors. Treating each hip as an unrelated observation can make the effective sample size look larger than it really is and can produce confidence intervals and p-values that are too optimistic.
The protocol should define the unit of analysis before data collection. Options include analysing one prespecified hip per patient, using patient-level outcomes, or including both hips with statistical methods that account for within-patient correlation. Patient-level risk factors should not simply be duplicated across two rows and analysed as though they arose from two independent people.
Yes, but the scope and format should be adapted. A PhD proposal usually requires a broader research gap, stronger novelty, a more detailed causal or prognostic framework and often longer follow-up than a single MS dissertation. Residency and board projects are generally narrower and should remain achievable during training.
For the Saudi Commission for Health Specialties and Saudi Board, the Arab Board of Health Specializations, and programmes linked to DHP, DHA, DOH or MOHAP, the same avascular necrosis question may be suitable when rewritten to match the relevant research-project template, supervision requirements, ethics pathway, institutional data-access rules and expected project scope. An Indian MS synopsis should not simply be copied unchanged into another board pathway.
Register after confirming the expected case volume, availability of baseline radiographs and MRI where required, consistency of staging and lesion measurements, access to the relevant treatment records and the feasibility of completing the chosen follow-up interval. For surgical studies, the number of patients actually receiving the specific procedure is more important than the total number of avascular necrosis patients seen in the outpatient department.
